1. Select the Add tab from menu on the left tool bar, then select Form.

2. Select a form template from the menu and click OK, Now you will be able to place the menu onto your website.

3. You will now be able to design and customise the form suit your needs. You can edit the form at any time using the Manage Form fields menu that will be accessible by selecting the form, then click manage form.
