1. Select the Add tab from menu on the left tool bar, then select Form.
2. Now you will be able to place the menu onto your website in the desired location.
3. You will now be able to design and customise the form to suit your needs. You can edit the form at any time using the 'Manage Form fields' menu that will be accessible by selecting the form, then click 'Manage Form'.
